Patient Assistance Fund
The Foundation for the Advancement of Clinical TMS (FACTMS) recognizes that there are often barriers for individuals to access TMS therapy. For example, barriers may transportation, need for local room and board, and need for childcare support. To help patients, FACTMS is offering grants up to $500 to TMS clinics to assist patients with TMS related expenses. Clinics may receive up to $1500 in grant funds annually, subject to availability, and grant funds may be distributed to one or more qualified patients.
Process
The clinic must submit a completed FACTMS Patient Assistance Fund application (Attachment A) to info@factms.org.
To meet eligibility, the patient’s income level must be less than or equal to 150% of the Federal Poverty Guidelines (See attachment B).
Funds can be used for any expense related to receiving TMS therapy.
A report documenting how funds were spent must be sent to the FACTMS office the quarter following distribution (See attachment C), including a clinician testimonial.
Upon receipt of approved distribution report, the clinic may reapply for additional funds up to two times within one calendar year.
